Application Wizard on a 64bit machine uses the folder
C:\Program Files (x86)\Visual DialogScript 5

But vds cannot use folders containing "(", ")".
So, you must change the name ...

You have reserved only one column for the table.
If you replace

DIALOG ADD,TABLE,TABLE1,10,7,223,144,Column 1|Column 2|Column 3

everything will be OK.
